Phase 1: Triage (Days 1-3)
Don't try to fix everything. Start by identifying what's most likely to draw surveyor attention.
- Credentialing: Pull five random practitioner files. Are they complete and current?
- Infection Control: Review sterilization logs for gaps in the last 90 days.
- QAPI: Can you show one completed improvement project with measurable results?
- Emergency Preparedness: When was your last drill, and is it documented properly?
- Governing Body: Do your board minutes reflect actual oversight?
Create a punch list. Prioritize by risk.
Phase 2: Fix and Document (Days 4-10)
Work the punch list systematically. For each gap:
- Fix the immediate issue.
- Document what you found and what you did.
- Assign ongoing ownership so it doesn't recur.

The Final 48 Hours
Stop fixing. Start stabilizing.
- Ensure leadership is available and accessible.
- Have a designated person to escort surveyors and retrieve documents.
- Pre-stage commonly requested files: licenses, certifications, recent meeting minutes.
- Remind staff: Be helpful. Be honest. Be calm.
